Job summary

Sequoia Transitional Care in Porterville, CA is seeking a Staff Physical Therapist to join our therapy department for full-time or part-time roles. The PT will evaluate and treat residents, collaborate with a multidisciplinary team, and support education for staff and families.

The ideal candidate holds a valid California PT license, CPR certification, and experience in geriatrics; strong documentation and communication skills are essential.

Qualifications

• Valid California Physical Therapy License is required.
• Current CPR certification is required.
• Experience in geriatrics or rehabilitation preferred.
• Strong communication and documentation skills required.
• Ability to relate well with patients, families, and interdisciplinary team.

Responsibilities

• Evaluate patients within 24 hours of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Deliver treatment per physician orders and PT best practices.
• Supervise Physical Therapy Assistants per state regulations.
• Maintain detailed, accurate documentation of patient care.
• Participate in interdisciplinary meetings and care conferences.
• Educate caregivers for post-discharge care.
• Assist with ordering of durable medical equipment (DME).
• Ensure department equipment is maintained in good working condition.
• Provide in-service education for facility staff.
