Brentwood Terrace Healthcare and Rehabilitation Center is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will assess residents, develop individualized treatment plans, and administer therapies to improve mobility and quality of life.

We value collaboration with an interdisciplinary team, thorough documentation, and compliance with all regulations. Texas licensure and LTC experience are preferred, with generous compensation and benefits on offer.

Formación

• Active Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy practice.
• Experience with Net Health EHR preferred.

Responsabilidades

• Consult with patients to assess condition and functional limits.
• Develop individualized treatment plans and rehabilitation programs.
• Administer PT treatments to relieve pain and restore function.
• Monitor patient response and adjust plans with the interdisciplinary team.
• Document comprehensively per policies and regulations.

Join Our Team as a Full-Time/PRN Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patients goals
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Physical Therapist license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of physical therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
