Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life, delivering therapies per individualized plans.

Your impact includes assessing patients, developing treatment plans, administering PT interventions, documenting thoroughly, and collaborating with an interdisciplinary team to ensure high-quality care and compliant operations.

Qualifications

• Active and valid Texas Physical Therapist license
• In good standing with all regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of physical therapy treatment practices

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Skilled Nursing Facility (SNF) or long-term care experience preferred

• Current knowledge of physical therapy treatment practices

• Experience with Net Health Electronic Documentation System preferred

• Ability to manage patients with varying personalities and needs

• Strong communication, collaboration, and clinical decision-making skills
