Summary

We are hiring a home health Physical Therapist for home visits! We are open to Full Time or Part Time in Jacksonville.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates the patient's progress.

Responsibilities

• Build trusted in-person relationships with patients.
• Provide continuing education and utilize tuition reimbursement opportunities.
• Work independently with autonomy.
• Explore career growth possibilities.
• Assess and evaluate patient needs, assist in developing the therapy plan of care, and monitor patient responses.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Educate patients, families, and healthcare team members about the plan of care, functional limitations, and progress.

Education and Experience

• Current Physical Therapy license in Florida.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
