Summary

We are hiring a home health Physical Therapist for home visits in Jacksonville. This position can be full-time or part-time.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

• Treat patients to relieve pain, restore or maintain function, using accepted standards of practice
• Educate patients, families, and healthcare team members about the care plan and progress
