We are hiring a home health Physical Therapist for home visits! We are open to Full Time or Part Time in Jacksonville.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve patient well-being, and evaluates patient progress.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health evaluates patient needs related to mobility, endurance, pain, and other conditions. They help develop treatment plans, perform interventions, and assess patient progress.

• Assess patient functional status and therapy needs; assist in developing the care plan with physicians
• Monitor and report patient responses and condition changes
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards
• Educate the healthcare team, patients, and families about the care plan and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in FL
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
