SUMMARY:

QUALIFICATIONS:This position requires a Physical Therapist with a current license in the State of Wisconsin/Illinois.
HEALTH REQUIREMENTS:May provide a health statement signed by a physician and must show evidence of a current negative tuberculin skin test or x-ray. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous positive T.B. test. Employee must be free of physical/medical conditions which would limit or restrict their ability to perform the job functions listed below.
ESSENTIAL FUNCTIONS: (Other duties as assigned)
1. Provides physical therapy services prescribed by the physician that can be safely provided in the home. Completes initial evaluation and admission visit within 48-72 hours of referral for patients being admitted for physical therapy and develops a plan of care.
2. Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test.
3. Supervises physical therapy assistants.
4. Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
5. Arranges the provision of services listed above on an outpatient basis which cannot be given in the patient's home.
6. Observes, records, and reports to the physician and other agency staff the patient's reaction to treatment and other changes in the patient's condition or plan of care.
7. Instructs patients and/or caregivers in the care and proper use of equipment and devices such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices.
8. Instructs family members and other health team personnel, including, when appropriate, home health aides, in certain phases of physical therapy so they can work with the patient.
9. Develops a patient care plan and measures progress or lack of progress in goal achievement.
10. Responsible for the physical therapy plan of treatment, daily physical therapy summaries,
and discharge summaries.
11. Participates in orientation of new employees and in-service programs related to physical therapy programs.
12. Participates in case conferences and meetings as needed.
I 3. The PT will maintain required hours of In-service training per calendar year, and attend mandatory In-services.
