Summary

This job description outlines the essential functions of the Physical Therapist position. It provides clarity for employees, administration, the personnel department, applicants, and other agencies regarding the role's responsibilities, organizational placement, and skill requirements. Qualifications

Licensed Physical Therapist in the states of Wisconsin or Illinois is required.

Health Requirements

Employees must submit a health statement signed by a physician and demonstrate evidence of a current negative tuberculin skin test or x-ray. An annual tuberculin skin test is required unless there is documentation of a previous positive test. The employee must be free from conditions that limit or restrict job performance.

Essential Functions
  1. Provide prescribed physical therapy services safely in the patient's home, including completing an initial evaluation and developing a care plan within 48-72 hours of referral.
  2. Assist physicians in evaluating patients through diagnostic and prognostic tests.
  3. Supervise physical therapy assistants.
  4. Treat patients to relieve pain, restore function, and maintain performance using physical methods like exercise, massage, heat, and water.
  5. Coordinate outpatient services that cannot be provided at home.
  6. Monitor and report patient reactions and condition changes to the physician and staff.
  7. Instruct patients and caregivers on equipment use, including wheelchairs, braces, and prosthetic devices.
  8. Educate family members and health team personnel involved in patient care.
  9. Develop patient care plans and evaluate progress towards goals.
  10. Manage treatment plans, daily summaries, and discharge documentation.
  11. Participate in orientation and in-service training programs.
  12. Attend case conferences and meetings as needed.
  13. Maintain required hours of in-service training annually and attend mandatory sessions.
