Job summary

Jackson Therapy Partners seeks a full-time Physical Therapist to provide home health services in the Seattle area. You will evaluate patients, implement plans of care, and work with a care team to optimize outcomes for patients with injuries or chronic conditions.

The role requires a WA license and relevant experience; supervision of PTAs and cooperation with home health aides is expected. A comprehensive orientation and ongoing CE are provided.

Qualifications

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Washington State PT license required to start.
  • BLS certification from AHA or ARC may be required.
  • Experience providing care in home health settings preferred.
  • Experience supervising PTAs and coordinating with home health aides preferred.
  • Familiarity with OASIS assessments, ICD‑10 coding, and documentation for home health required.
  • Ability to manage a typical caseload of approximately 25 visits per week (20 visits/week when paired with a PTA).

Responsibilities

  • Provide treatments according to physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
  • Work with home care personnel in patient care planning.
  • Direct and supervise Physical Therapy Assistants and Home Health Aides, as assigned.
  • Take initial patient history and perform initial evaluations.
  • Consult with physicians regarding changes in treatment.
  • Instruct patients and families in home programs and activities of daily living.
  • Participate in Quality Assessment and Performance Improvement activities as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documentation for significant changes in condition.
  • Appropriately utilize ICD‑10 codes in documentation.
