Job summary

Jackson Therapy Partners is seeking a full-time Physical Therapist to join our home health team in the Renton, WA area. The role focuses on helping patients move better, feel better, and stay active, whether recovering from injury or managing chronic conditions.

Responsibilities include supervising PTAs/HHAs, evaluating patients, coordinating with physicians, and completing OASIS and ICD-10 documentation. A WA PT license and BLS are required or preferred, with travel within the region.

Qualifications

  • Licensed Physical Therapist in Washington with a PTA-approved degree.
  • Active Washington State PT license required to start.
  • BLS certification from AHA or ARC may be required.
  • Home health experience preferred.
  • Ability to perform OASIS assessments and documentation.
  • Competence with ICD-10 coding.
  • Experience supervising PTAs and HHAs preferred.
  • Willingness to cover Redmond area and travel within the region.

Responsibilities

  • Provide physician-ordered care and implement plan of care under supervision.
  • Collaborate with home care personnel on patient care planning.
  • Direct and supervise Physical Therapy Assistants and Home Health Aides.
  • Take initial patient history and perform initial evaluations.
  • Consult with physicians on treatment changes.
  • Instruct patients and families in home programs and daily living activities.
  • Participate in QA/PI activities and continuous improvement.
  • Complete and submit required OASIS documentation with ICD-10 coding.

Job description

We’re looking for a Physical Therapist to take on a full-time role with one of our area clients. You’ll help patients move better, feel better, and stay active—whether they’re recovering from an injury or managing long-term conditions.

Minimum Requirements
  • Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
  • Active Washington State Physical Therapy license is required to start the position.
  • BLS certification from AHA or ARC may be required.
  • Home health experience preferred or prior experience working in home health or community-based settings.
  • Ability to complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
  • Competence with ICD-10 coding for appropriate documentation.
  • Experience supervising PTAs and Home Health Aides preferred.
  • Willingness to cover the Redmond area as part of the caseload and travel within the assigned region.
