Job summary

Ideal candidates have CAPTE-approved PT education and WA licensure, plus 2 years of PT experience; home health experience is a strong plus. OASIS experience and a valid driver's license are preferred requirements.

Qualifications

Responsibilities

• Conduct initial assessments, develop plans of care, and reassess every 30 days.
• Provide physician-prescribed PT in patients' homes.
• Perform skilled procedures as ordered by the physician.
• Complete and submit OASIS assessments and documentation timely.
• Use ICD-10 codes for documentation.
• Prepare clinical and progress notes accurately and promptly.
• Consult with physicians on treatment changes and write progress reports.
• Instruct patients and families in home programs and ADLs.
• Participate in care planning and conferences with home care staff.
• Assist physician in evaluating function and revising the plan of care.
• Direct and supervise PTAs and HHAs as assigned.
• Participate in in-service programs and QA/QI activities.

Physical Therapist – Home Health | Seattle, WA

We are partnering with a well-established home health agency serving the King County, Washington area to find a skilled and compassionate Registered Physical Therapist (PT) to join their dedicated home health team. This is a permanent, full-time opportunity for a clinician who values autonomy, meaningful one-on-one patient relationships, and the ability to make a real difference in patients' lives within their own homes.

Position Details

• Setting: Home Health (patient homes throughout Seattle, WA area)
• Patient Population: Adult / Geriatric — homebound patients
• Schedule: Full-Time | Flexible / field-based
• Location: Seattle, WA
• Pay Rate: $50.00/hr – $67.45/hr DOE
• Start Date: ASAP

About the Role

As a Home Health Physical Therapist, you will administer physician-prescribed physical therapy to patients on an intermittent basis in their place of residence, in accordance with the plan of care and under the direction and supervision of the Director of Clinical Services/Clinical Manager. You will work to improve or minimize residual physical disabilities and return individuals to their optimum functional level within their capabilities.

Key Responsibilities

• Conduct initial assessments, develop plans of care, and perform reassessments every 30 days
• Provide physician-prescribed physical therapy in patient homes
• Perform all skilled procedures as ordered by the physician
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change-in-condition documentation within Agency-defined timeframes
• Utilize appropriate ICD-10 codes for all documentation
• Prepare clinical and progress notes accurately and in a timely manner
• Consult with physicians regarding changes in treatment and write progress reports
• Instruct patients and family/significant others in home programs and activities of daily living
• Participate with all home care personnel in patient care planning and care conferences
• Assist physician with evaluating level of function and help develop/revise the plan of care
• Direct and supervise Physical Therapy Assistants and Home Health Aides as assigned
• Participate in in-service programs and Quality Assessment and Performance Improvement activities

Requirements

• Graduate of a PT program approved by CAPTE, APTA, or equivalent accrediting body — required
• Current Washington State PT license in good standing — required
• Minimum 2 years of experience as a Physical Therapist — preferred
• Home health or field-based clinical experience — a strong plus
• OASIS documentation experience preferred
• Ability to exercise initiative and independent judgment
• Valid driver's license and reliable transportation — required
• Strong communication, organization, and time management skills
