Job summary

Country Manor Campus LLC is seeking a Full Time Inpatient Physical Therapist in Sartell, MN, offering a $5,000 sign-on bonus. Join an award-winning therapy team in an inpatient setting with opportunities to learn and grow as part of an interdisciplinary group.

Responsibilities include direct patient care, plan-of-care adjustments, and coordination with physicians to ensure smooth discharge. Eligible new grads encouraged to apply and MN licensure is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Currently licensed to practice in MN or eligible for licensure.
• New graduates and temporary license holders encouraged to apply.

Responsibilities

• Direct client care: continuous assessment of patient needs and functional deficits, treatment, and plan-of-care modifications.
• Collaborate with interdisciplinary team to maximize patient outcomes.
• Provide clinical education to families and staff; guide safe and successful discharge; supervise PT Assistant; communicate with physicians.
• Complete required documentation per state and federal guidelines.

Champion Rehab is looking for a compassionate, innovative and energetic Physical Therapist to join their award-winning therapy team!

This position offers the unique opportunity to gain experience and knowledge within an inpatient setting.

Responsibilities:

• Direct client care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed
• Willingness to work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
• Applying clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Physical Therapy Assistant, communicating with Physicians to ensure fluency of care
• Documentation which follows regulatory guidelines (state, federal)

Required Qualifications:

• Graduate of an accredited Physical Therapy program
• Currently licensed to practice in State of MN. New graduates and temporary license holders encouraged to apply.
