Job summary

Aveanna Healthcare in Minnesota is seeking a dedicated Physical Therapist to assess, teach, and deliver home-based therapy interventions directed by the physician and outlined in the plan of care.

You will collaborate with an interdisciplinary team, perform ongoing mobility and function assessments, document progress, coordinate discharge planning, and supervise support staff as applicable—enabling patient recovery and independence.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in MN or applicable state.
• Valid CPR certificate, driver's license and automobile insurance.

Responsibilities

• Conduct initial and ongoing mobility assessments to set goals.
• Administer tests and measurements to determine functional outcomes.
• Document evaluation and plan of care with progress notes.
• Deliver therapy in accordance with physician orders.
• Coordinate discharge planning with patient and caregivers.
• Supervise PTA/OTA/HHA staff as applicable.
• Complete OASIS assessments where state allows.
• Maintain communication with health care team and family.

We are looking for a Physical Therapist (PT) to join our team! You will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the Physician as established in the home health plan of care. You will collaborate with interdisciplinary team members, receiving and providing pertinent updates to optimize patient well‑being.

Full‑Time Visits

Full‑time visits needed in Garrison, Onamia, Wahkon, Isle, Aitkin, Deerwood, and Crosby, MN communities.

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Provide written evaluation and plan of care defining PT needs, goals, progress and expected outcomes.
• Deliver physical therapy services in accordance with prescribed physician orders.
• Maintain plan of care by timely documentation of client performance, modify goals and treatment as needed, and determine discharge care plans as appropriate.
• Plan and coordinate discharge, including educating patient and caretakers on home exercise plans.
• Participate in coordination of services with healthcare team, physicians, parent/guardians, nurses, and others.
• Document according to policy and procedure, resulting in no technical or clinical denials by payers.
• Maintain effective communication between staff, healthcare team members and family; serve as resource consultant for clinical staff.
• Supervise other personnel (PTA, OTA, HHA) as applicable.
• Complete OASIS assessments where allowed by state professional practice and regulation.
• Follow policies, procedures and comply with all applicable patient care standards, licensure regulations and state laws.

Requirements

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice.
• Valid CPR certificate, driver's license and automobile insurance.
