Job summary

Alpha Home Health and Hospice seeks a Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans for adult and geriatric patients with orthopedic, neurological, and functional impairments. Join a patient-centered team delivering care in home settings.

You will perform evaluations, design plans of care, supervise rehab staff, educate on home programs, and participate in ongoing professional development to improve outcomes.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Current Missouri Physical Therapist license or eligibility.
• Valid Basic Life Support (BLS) certification.

Responsibilities

• Perform comprehensive patient evaluations and develop age-appropriate plans of care.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Provide skilled interventions such as therapeutic exercises, mobility training, and balance activities.
• Reassess patient progress and modify treatments accordingly.
• Document treatment activities, patient progress, and discharge summaries accurately and timely.
• Supervise rehabilitation technicians and Physical Therapist Assistants per licensure standards.
• Educate patients and caregivers on home programs, safety, and ongoing rehabilitation needs.
• Participate in professional development, departmental meetings, and quality initiatives.
