Summary

We are currently hiring a full-time Physical Therapist for home health in Naples!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Key Responsibilities

• Assess and evaluate patients' functional status and physical therapy needs, assisting physicians in developing the therapy plan of care
• Monitor and report patients' responses to treatment and any condition changes to the supervising nurse or physician
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to accepted standards
• Educate patients, families, and healthcare team members about the care plan, functional limitations, and progress

Education and Experience

Licensure Requirements:

• Current Physical Therapy license in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
