Summary

We are currently hiring a full-time PT for home health in Naples!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Responsibilities

• Evaluate patients' functional status and physical therapy needs; assist physicians in developing the therapy plan of care.
• Monitor and report patient responses to treatment and any condition changes to supervising nurses and/or physicians.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards of practice.
• Educate patients, families, and healthcare team members about the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements:

• Current Physical Therapy license in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
