Summary

We are hiring a Full-time PT for Home Visits in the south part of Brevard County - Viera, Melbourne, or beachside around Cape Canaveral to Palm Bay!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps determine a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates the patient's progress.

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for:

• Evaluating the patient's functional status and physical therapy needs, assisting in developing the therapy plan of care with the physician.
• Monitoring and reporting the patient's response to treatment and any condition changes to the supervising nurse or physician.
• Treating patients to relieve pain, restore function, and maintain maximum functioning according to accepted standards.
• Educating healthcare team members, patients, and families about the plan of care, functional limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
