Summary

We are hiring a full-time PT for home visits in the south part of Brevard County - Viera, Melbourne, or beachside around Cape Canaveral to Palm Bay.

The Physical Therapist in home health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps develop a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities

• Assess and evaluate patients' functional status and needs, assisting physicians in developing care plans.

• Educate patients, families, and healthcare team members about care plans, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
