We are hiring a full-time PT for home visits in the south part of Brevard County - Viera, Melbourne, or beachside around Cape Canaveral to Palm Bay!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Responsibilities:

• Assess and evaluate patient needs related to mobility, endurance, pain, and other conditions. Assist physicians in developing the therapy plan of care.
• Monitor and report patient responses and condition changes to the healthcare team.
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
• Educate patients, families, and healthcare team members about the care plan, limitations, and progress.

Education and Experience:

Licensure Requirements:

• Current Physical Therapy license in Florida
• CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
