Job summary

Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a Physical Therapist (PT) to support our therapy team as needed on the Aurora, OH campus. The position offers FT or PT shifts with a pay range of $38.00–$50.00 hourly based on experience.

The PT is responsible for evaluating residents and implementing individualized plans of care to improve mobility, strength, balance, and independence within a supportive, family-owned environment.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled physical therapy treatment services in accordance with physician orders
• Collaborate with and supervise PTAs as applicable
• Accurately document evaluations, treatments, progress notes, and discharge planning
• Participate in interdisciplinary care planning and team communication
• Ensure compliance with facility policies, professional standards, and regulatory requirements
