Job summary

Family Tree Rehabilitation, part of The Campus of Anna Maria, on a wooded Ohio campus, is seeking a Physical Therapist (PT) to support our therapy team as needed. This family-owned skilled nursing facility provides resident-centered rehabilitation services in a collaborative environment.

The role includes evaluating residents, developing plans of care, providing therapy per physician orders, and documenting progress.

Qualifications

• Current Ohio Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing or rehabilitation setting preferred.
• Strong clinical judgment, communication, and time‑management skills.
• Compassionate, resident‑centered approach to care.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care.
• Provide skilled physical therapy treatment services in accordance with physician orders.
• Collaborate with and supervise PTAs as applicable.
• Accurately document evaluations, treatments, progress notes, and discharge planning.
• Participate in interdisciplinary care planning and team communication.
• Ensure compliance with facility policies, professional standards, and regulatory requirements.
