Job summary

Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a Physical Therapist to support our therapy team on a full- or part-time basis. Located on a wooded campus in Ohio, this family-owned facility emphasizes resident-centered rehabilitation in a collaborative environment.

The PT will evaluate residents, develop and implement plans of care, and help improve mobility, strength and independence while collaborating with the interdisciplinary team and supervising PTAs as applicable.

Qualifications

• The Physical Therapist evaluates residents, develops and implements individualized plans of care, and helps residents improve mobility, strength, balance, and independence.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled physical therapy treatment services in accordance with physician orders
• Collaborate with and supervise PTAs as applicable
• Accurately document evaluations, treatments, progress notes, and discharge planning
• Participate in interdisciplinary care planning and team communication
• Ensure compliance with facility policies, professional standards, and regulatory requirements

Position Summary
