Role Overview:

Services are provided across the continuum, focusing on quality, cost-effectiveness, and regulatory compliance.

Key Responsibilities:

• Treatment Planning & Care: Implements treatment plans, selects appropriate activities, revises goals with patient input, provides education, discharge instructions, and referrals.
• Patient Assessment: Collaborates with physicians to establish and evaluate treatment programs, goals, and discharge plans, ensuring patient and family involvement.
• Documentation: Records evaluations, treatment sessions, education, and discharge details per departmental guidelines.
• Professional Conduct & Customer Service: Upholds ethical standards, maintains confidentiality, communicates effectively, fosters teamwork, and promotes a positive environment.
• Development & Compliance: Participates in ongoing education, completes required competencies, and adheres to organizational policies and infection control protocols.
