Job summary

Crystal Lake Healthcare & Rehabilitation in Bayville, NJ is seeking a compassionate Physical Therapist to join our rehabilitation team. We offer Full-Time or PRN positions to evaluate and treat residents in skilled nursing and post-acute rehab programs, ensuring individualized care and strong teamwork with nurses and physicians.

The ideal candidate holds an Associate Degree in Physical Therapy, a New Jersey PT license, and a commitment to ongoing education.

Qualifications

• Associates Degree or higher in Physical Therapy from an accredited program
• Active New Jersey Physical Therapy license
• Ongoing commitment to continuing education and professional development
• Long-term care experience is a plus
• Strong organizational, communication, and patient care skills

Responsibilities

• Conduct comprehensive evaluations to determine therapy needs
• Develop and implement individualized treatment plans
• Provide therapy interventions to improve mobility, function, and independence
• Educate residents and families on exercises and treatment plans
• Document progress and adjust therapy plans as needed

Position Summary

As a Physical Therapist, you will evaluate and treat residents to improve mobility, strength, and overall quality of life. You will work closely with nurses, physicians, and other rehab staff to create and implement individualized treatment plans, ensuring residents receive the best care possible.
