Job summary

The Savoy at Fort Lauderdale Rehabilitation & Nursing Center is seeking a Physical Therapist (PT) to join our growing team. You will assess referred patients and create individualized rehabilitation plans focused on improving function and independence.

You will document progress, educate patients and families, supervise PT Assistants, and collaborate with other disciplines to ensure top-quality, patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid Florida physical therapy license and registration.
• Experience with long-term care or geriatric care preferred.
• Strong documentation writing skills and charting experience.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines for patient-centered care.
• Professionalism, punctuality and confidentiality remain essential.
• Overseeing and supervising Physical Therapy Assistants.
• Comply with HIPAA and Residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation services and determine needs.
• Develop and administer treatment plans with goals based on patient capacity.
• Set realistic goals, monitor progress and adjust plan as needed.
• Educate patients and families on therapy methods and expectations.
• Record patient condition, progress and discharge plans accurately.
• Collaborate with other disciplines to provide best care and service.
