Tender Touch Rehab Services LLC in Tallahassee, FL is seeking a licensed Physical Therapist for a full-time role in a Skilled Nursing Facility setting. You will evaluate and treat patients with mobility, strength, balance, and functional impairments while developing personalized care plans.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• Minimum 1 year SNF experience and willingness to travel between facilities.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
