Renewal Rehab, part of Enhance Therapies, is hiring a licensed Physical Therapist for a full-time role in a skilled nursing facility in Jacksonville, Illinois. You will evaluate patients, design personalized care plans, and work with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements; ensure all physician orders are obtained and up to date.
