Job summary

Tender Touch Rehab Services LLC is seeking a Physical Therapist Floater for full-time to join our Skilled Nursing Facility and Long-Term Care team in New Jersey. You will evaluate and treat patients with mobility, strength, balance, and functional impairments, and create personalized care plans.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of New Jersey.
• Strong communication and organizational skills.
• One year SNF experience and willingness to travel to multiple facilities.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
