Tender Touch Rehab Services LLC is seeking a licensed Physical Therapist Floater for a Full-Time role in our Skilled Nursing Facility network. You will evaluate patients, develop personalized care plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Must hold an active Physical Therapist license in the state of practice.
• Degree from an APTA-accredited program (BS, MS, or DPT).
• Minimum one year of SNF experience; willing to travel to multiple facilities.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
