Job summary

Bronte Health and Rehab Center in Bronte, Texas is seeking a licensed Physical Therapist to provide one-on-one therapy sessions and help SNF residents and outpatient clients achieve rehabilitation goals. You will assess patient needs, develop and implement personalized treatment plans, and collaborate with the care team to ensure optimal outcomes.

The role requires a Master's in Physical Therapy and current Texas licensure, strong communication, and the ability to work with a diverse patient

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current licensure as a Physical Therapist in Texas.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct initial and ongoing assessments of patients' physical function and limitations.
• Develop and implement individualized treatment plans to address PT needs.
• Provide one-on-one therapy sessions (exercises, stretches, and interventions).
• Collaborate with physicians, occupational therapists, and speech therapists.
• Maintain accurate records of patient progress and treatment plans.
• Participate in patient care conferences and care plan development.
