Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC is seeking a Physical Therapist to join our long-term care team. The role involves comprehensive PT evaluations, creating and updating treatment plans, and coordinating with the interdisciplinary team to meet resident goals.

Qualifications

• Requires graduation with a degree in Physical Therapy (B.S./DPT/MS/MA) recognized by the APTA.
• Holds current license/registration in the state (as applicable).

Responsibilities

• Provide comprehensive PT evaluations based on MD orders.
• Develop and document treatment plans with short and long-term goals.
• Ensure orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver patient-centered therapy with appropriate modalities and safety precautions.
• Document daily participation and progress; update treatment plans and discharge summaries.
• Co-sign notes and supervise junior staff or students as needed.
