Qualifications

• DPT or PT Master's degree and NY PT license or eligibility.

Responsibilities

• Evaluate and treat patients in SNF settings, focusing on orthopedic, neuro, cardiac, and deconditioning recovery.
• Develop individualized treatment plans and document progress.
• Collaborate with nursing, physicians, and families to optimize discharge outcomes.
