Job summary

Twin Fountains in Georgia is seeking a licensed Physical Therapist to join our caring rehab team. You will perform comprehensive PT evaluations, develop and implement treatment plans, and coordinate with MD orders for goals, frequency, and modalities in a long-term care setting.

We offer a supportive work environment with training, resources, and a schedule that fits a flexible, Monday–Friday 8-hour shift, focusing on geriatrics and patient-centered care.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
