Job summary

Rockwell Park Rehabilitation and Healthcare Center in Charlotte, NC, is hiring a part-time Physical Therapist to evaluate residents, develop treatment plans, and provide therapy services according to MD orders.

You will document progress, collaborate with an interdisciplinary team, ensure compliance, and participate in care planning and staff education. Qualified PT with state license and APTA-recognized degree preferred.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license/registration in the state as applicable.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form adhering to guidelines.
• Provide a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment utilizing modalities and modify as indicated while adhering to precautions.
• Document daily participation and progress on weekly progress note.
• Update treatment plan and document discharge and in-services as needed.
• Complete documentation for the facility and information needed for the MDS.
• Attend facility and rehab meetings and participate in committee activities.
• Act as a rehab delegate to the interdisciplinary team and participate in scheduling.
• Perform screens and complete billing and billing accuracy for PT services.
• Cover other therapists on caseload during absence and supervise junior staff or students as needed.
• Maintain professional relationships with staff, patients, and families.
• Provide in-service education to rehab staff, facility staff, hospitals and community as needed.
• Comply with facility policies and patient confidentiality.
