Job summary

Eden Rehabilitation And Healthcare Center in Eden, NC is hiring a part-time Physical Therapist to evaluate and treat residents per physician orders, collaborating with an interdisciplinary team to plan care.

You will document evaluations and progress, develop treatment plans, and ensure compliant care while delivering high-quality therapy alongside supportive staff. A valid PT license in North Carolina is required. Flexible schedule offered.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated.
• Document daily participation and weekly progress.
• Document discharge and in-services on a discharge summary.
• Complete billing and bill patients ethically and accurately for physical therapy services rendered.
• Supervise junior staff members or students as needed.
