Patriot Health and Rehab is seeking a Physical Therapist (PT) to join our Therapy Team to plan, organize, develop, and direct physical therapy services in accordance with applicable standards and regulations. A $3,000 sign-on bonus is offered.

Qualifications

• Bachelor of Science Degree in Physical Therapy is required.
• Active Physical Therapy license in TN is required.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Document and bill for physical therapy services per CMS expectations.
• Develop and implement treatment plans with short- and long-term goals.
• Communicate findings and plans to residents, families, and team members.
