Job summary

Resolve Solutions is seeking a Full-Time Physical Therapist to join our skilled nursing therapy team on an as-needed basis, providing high-quality rehabilitation in a SNF setting.

The role requires an active Wisconsin PT license, graduation from an accredited program, and willingness to work weekdays and/or weekends as facility needs. Strong communication and documentation skills are essential to support resident goals and discharge planning.

Qualifications

• Licensed Physical Therapist in Wisconsin (WI).
• Graduated from an accredited Physical Therapy program.
• SNF or post-acute care experience preferred; new graduates welcome.
• Strong communication, documentation, and time-management skills.

Responsibilities

• Perform evaluations and develop individualized physical therapy treatment plans
• Supervise PTA treatment when applicable
• Collaborate with interdisciplinary teams to support resident goals and discharge planning
• Provide skilled therapy services to short-term rehab and long-term care residents
• Complete accurate and timely documentation in accordance with Medicare and facility guidelines
