Job summary

Resolve Solutions- West Allis is seeking a Full-Time Physical Therapist to support our skilled nursing therapy team in a SNF setting, offering flexible scheduling to meet facility needs.

You will evaluate residents, develop individualized treatment plans, supervise PTA treatment when applicable, collaborate with interdisciplinary teams, provide skilled therapy for short- and long-term residents, and document in line with Medicare and facility guidelines.

Qualifications

• Active Physical Therapist license in Wisconsin (WI).
• Graduate of an accredited Physical Therapy program.
• SNF or post-acute care experience preferred; new graduates welcome.

Responsibilities

• Perform evaluations and develop individualized physical therapy treatment plans
• Supervise PTA treatment when applicable
• Collaborate with interdisciplinary teams to support resident goals and discharge planning
• Provide skilled therapy services to short-term rehab and long-term care residents
• Complete accurate and timely documentation in accordance with Medicare and facility guidelines
• Participate in care planning and interdisciplinary communication as needed
