Qualifikationen

• Wyoming Physical Therapist license (active or eligible).
• Bachelor's degree in Physical Therapy.
• Complete an accredited Physical Therapy program.
• Active BLS certification.
• Experience with adult patients; pediatric or school-based settings not preferred.
• Ability to work independently with minimal supervision.
• Strong time-management and organizational skills.
• Strong communication, clinical judgment, and patient-care skills.

Aufgaben

• Therapists will work with residents in the Intermediate Care Facility and SNF.
• Collaborate with the IDT to develop and implement care plans.
• Perform therapeutic evaluations including initial, quarterly, and annual assessments.
• Evaluate and assist in the design and purchase of adaptive equipment.
• Provide therapeutic treatment and activities.
• Consult with medical providers, families, case managers, and staff regarding residents’ needs.
• Train, educate, and monitor staff in therapeutic skills and interventions.
• Complete timely documentation.
