Job summary

Home&Care in Casper, WY is seeking a full-time Physical Therapist to deliver skilled PT services in patients' homes, building meaningful relationships and promoting independence. You will assess, plan, and implement rehabilitation aligned with physician goals while coordinating with the care team and supervising PTAs.

Candidates must have an APTA-approved PT degree, Wyoming license, CPR certification, and strong communication and leadership skills.

Qualifications

Degree from an accredited Physical Therapy program (APTA-approved).
Current Wyoming PT license (unrestricted).
Current CPR certification.
Strong communication, organizational, and leadership skills.

Responsibilities

Provide skilled physical therapy treatments to patients with a variety of injuries, illnesses, and functional limitations to help them reach their highest potential.
Conduct assessments, record findings, and develop individualized treatment programs with measurable goals.
Design and implement rehabilitation programs that align with physician plans and patient goals, adapting as needed for optimal results.
Collaborate with physicians and the care team to update and refine treatment plans.
Supervise and provide guidance to Physical Therapy Assistants to ensure high-quality care delivery.
Maintain timely, accurate documentation of patient care and progress.
Communicate effectively with patients, families, and team members, reporting progress and updating care plans as needed.
