Job summary

Regal Heights Healthcare and Rehabilitation Center in Hockessin, Delaware, is seeking a full-time Physical Therapist to assess, plan, and deliver rehabilitation programs for residents. New grads are welcome and a $3,000 sign-on bonus is offered.

The role includes supervising staff, documenting care, coordinating with physicians and care teams, and supporting transition planning to appropriate levels of care. A CAPTE-approved PT program and DE license are required.

Qualifications

• Graduate of a CAPTE-approved Physical Therapy program.
• Valid Delaware Physical Therapy license.

Responsibilities

• Supervise Physical Therapy Assistants, PTAs and staff per licensure.
• Perform screens, evaluations, progress notes, and reevaluations in a timely manner.
• Establish short- and long-term goals with resident, physician, and care team.
• Obtain MD orders for evaluations, treatments, and discharges.
• Document rehab services and ensure plan of care is followed.
• Participate in care planning meetings and related activities.
• Travel to facilities within a reasonable distance as needed.
