Qualifications

• Valid state licensure as a Physical Therapist.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• CPR certification; other state-required credentials as applicable.

Responsibilities

• Conduct comprehensive assessments to determine functional abilities, goals, and treatment needs.
• Develop and implement personalized PT treatment plans with evidence-based modalities.
• Administer therapeutic exercises, manual therapy, and modalities to improve mobility and function.
• Maintain accurate, timely documentation of progress notes, evaluations, and plans of care.
• Educate patients and families on home programs, safety, and prevention techniques.
• Collaborate with physicians, nurses, and allied health professionals for integrated care.
