Job summary

Willow Springs Healthcare Center (Desert Springs) is seeking a compassionate Physical Therapist (PT) to join our growing therapy department. The PT will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, strength, balance, endurance, and functional independence.

The role requires graduation from an accredited PT program and a current CA PT license in good standing; CPR certification preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license in good standing.
• Current CPR certification preferred.
• Skilled nursing facility experience preferred but not required.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively in a team-oriented healthcare environment.
• Commitment to providing exceptional resident care and outcomes.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders.
• Provide skilled physical therapy services to residents in accordance with established plans of care.
• Assess mobility, balance deficits, strength impairments, pain, and fall risk; address these areas.
• Establish realistic, measurable treatment goals and modify plans as needed.
• Supervise and collaborate with PTAs and rehabilitation staff.
• Complete timely documentation in compliance with facility, state, and federal requirements.
• Communicate resident progress with physicians, nursing staff, families, and the team.
• Educate residents and caregivers on exercises, safety techniques, and discharge recommendations.
