Job summary

Willow Creek Health Care Center seeks a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a per-diem basis. Working under a licensed Physical Therapist, you will help residents improve mobility, strength, balance, safety, and independence in a supportive skilled nursing setting.

Responsibilities include administering PT treatments per plans, documenting progress, communicating with supervising PT, and collaborating with the care team to ensure optimal outcomes.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program required.
• Current California Physical Therapist Assistant license required.
• License must remain active, unrestricted, and in good standing.

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Follow individualized plans of care and established treatment protocols.
• Observe and document residents' responses to treatment and progress toward goals.
• Complete timely, accurate documentation in accordance with facility and payer requirements.
• Communicate regularly with the supervising Physical Therapist regarding resident progress and discharge planning.
• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, families, and other team members.
