Job summary

Hartford HealthCare in Connecticut seeks a Physical Therapist to administer medically prescribed therapies, evaluate functional status, and deliver services to maximize independence and reduce pain.

Open to new graduates; CT license required or temporary license at time of hire; prior PT experience preferred. You will collaborate with Rehab Therapists, Nurses, and Social Workers to support patient transitions.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy (CAPTE accredited)
• CT Physical Therapy license required or active CT temporary license at time of hire
• Open to new graduates; previous PT experience preferred

Responsibilities

• Thorough examination and evaluation of patient’s function and mobility after physician referral
• Establish measurable goals with the patient/family based on care protocols and critical paths
• Implements and delegates care with modalities and therapeutic exercise
• Provides patient and caregiver education regarding treatment and equipment use
• Coordinates care transitions and discharge planning with Rehab Therapists, Nurses, Social Workers and others
• Delegates care tasks and provides supervision for PT students and Rehab Aides
