Job summary

Cambridge Healthcare Services, LLC seeks a licensed Physical Therapist to evaluate, plan, and implement individualized therapy programs for residents in a skilled nursing facility.

You will collaborate with an interdisciplinary team to improve strength, mobility, balance, and quality of life, document progress, educate residents and families, and ensure compliance with regulatory requirements.

Qualifications

• DPT or MS in physical therapy from an accredited program.
• Current state licensure as a Physical Therapist.

Responsibilities

• Conduct thorough evaluations of residents' physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Monitor and document residents' progress and adjust treatment plans as necessary.
• Communicate effectively with residents and their families regarding treatment plans and progress.
• Maintain accurate and timely documentation of all patient interactions and treatments.
• Ensure compliance with federal, state, and local regulations and facility policies.
• Participate in quality assurance and performance improvement activities.
