Job summary

Cambridge Healthcare Services, LLC is seeking a skilled Physical Therapist to evaluate and treat residents in a skilled nursing facility. You will develop individualized plans aimed at improving strength, mobility, balance, and overall function, while maintaining dignity and respect for residents.

Collaborating with physicians, nurses, and therapists, you will document progress, ensure regulatory compliance, and educate residents and families on exercises to perform at home.

Qualifications

• Doctorate or master's degree in physical therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing facility preferred; new graduates encouraged.

Responsibilities

• Evaluate residents' physical abilities and limitations.
• Develop and implement individualized treatment plans.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Monitor and document residents' progress and adjust treatment plans as necessary.
• Work closely with physicians, nurses, and occupational therapists.
• Communicate effectively with residents and families regarding treatment plans and progress.
• Participate in interdisciplinary team meetings and care planning sessions.
