Job summary

Hillcrest Health & Living's Hillcrest Rehab Services is seeking a compassionate Part-Time Physical Therapist to join our interdisciplinary rehabilitation team. You will evaluate patients, develop evidence-based plans, and help restore movement and independence in a long-term care setting.

The role emphasizes collaboration with physicians, nurses, and therapy staff, supervision of assistants, documentation, and safe patient handling. A Nebraska PT license and BLS are required.

Qualifications

• Graduate of an accredited Physical Therapy program required.
• Current Nebraska Physical Therapist license or applicable compact state required.
• American Heart Association Basic Life Support (BLS) certification required.
• At least two years of Physical Therapy experience in long-term care, post-acute care, hospital, home care, or a similar setting preferred.
• ABPTS board certification in geriatric, orthopedic, or neurological physical therapy preferred.

Responsibilities

• Evaluate and treat patients with functional and mobility needs; develop individualized plans of care.
• Complete comprehensive physical therapy evaluations using tests and measures.
• Develop individualized plans of care based on patient needs and goals.
• Provide evidence-based interventions to improve mobility, strength, balance, function, and independence.
• Progress treatment plans based on patient response and goals.
• Educate patients and families about treatment plans, exercises, mobility, safety, and ongoing care needs.
• Collaborate with physicians, nurses, therapy team, and other disciplines to coordinate patient care.
• Supervise, delegate to Physical Therapist Assistants and therapy staff.
• Develop transition and discharge plans; ensure effective handoff between care settings.
• Identify patients who may benefit from additional services and complete appropriate screenings.
• Manage patient schedule considering preferences and clinical needs.
