Sobe Innovative Rehab in Winter Haven, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy services in a home setting on a PRN basis.

Responsibilities include evaluating and treating residents, developing individualized treatment plans, collaborating with physicians and families, and maintaining accurate EMR documentation. Qualifications
