Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy services in a home setting in Titusville, FL. You will evaluate and treat residents, develop individualized plans, and collaborate with physicians and families to maintain independence and quality of life.

• Active Florida Physical Therapist (PT) license.

Responsibilities

• Evaluate and treat residents in a home setting or assisted living as applicable.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
